TWIN SEPARATIONS: A HISTORY OF RISK

The death of baby Hope following an operation to separate her from her
sister is a reminder that despite advances, the outcome of conjoined
twin surgery remains unpredictable.

Even in the modern era, these operations are among the most challenging
for doctors, with painstaking preparation involving large teams
of specialists.

Only a handful of hospitals in the world have the resources and skills
needed to attempt separation.

Yet the history of such operations stretches back more than a century.

Conjoined twins are created just a few days after they are conceived –
most likely by the incomplete splitting of the fertilised egg.

Most are stillborn, and a proportion of those who a born alive do
not survive long afterwards.

They can be joined at the hip, the back, the chest, or even the head,
perhaps sharing one or more internal organs, each adding to the
complexity of any operation to separate them, as the surgeon seeks
to provide each with enough to support life.

In past centuries, when separation was frequently not an option,
there are many references to twins who grew up still joined.

These included two pairs reputedly born in Rome in the fourth and
seventh centuries AD and Armenian twins joined at the abdomen in the
tenth century AD, alongside the so-called "Biddenden Maids" from
England, who were joined at the spine, and reportedly lived into
their 30s.

The term "Siamese twins", now discarded, refers to Chang and Eng
Bunker, who found fame as circus performers.

Early operations

While there are reports of unsuccessful attempts to separate twins in
the 1600s, what is thought to be the first successful twin separation
was carried out in 1689, on twins joined only by a ligament at
the chest.

While operations on other twins with comparatively simple anatomies
were also successful, later attempts to separate those with more
shared organs and tissues were frequently only partially successful.

One example is in 1900, with chest-joined Brazilian sisters Maria
and Rosalina separated, only for Maria to die within days.

However, by the 1950s, specialists were becoming far more practised,
with the first recorded successes in separating twins joined at
the skull.

US surgeon Dr C. Everett Koop also managed to separate two girls who
shared a bowel, urinary system and genitals – one even managed to go
on to mother her own children.

‘Preparation key’

Professor Alastair Millar, from the Red Cross War Memorial Children’s
Hospital in Capetown, heads a centre responsible for dozens of
successful operations in recent years.

He said the key to success in recent years was the ability to plan
complex operations well in advance.

"You can now create 3D models on the computer to work out just how
you are going to do these operations.

"We are just getting better at everything – the surgical team has
been able to discuss what to expect and what role each will play."

However, he said: "In the end, it might not matter how extensively
you have prepared, there can always be a surprise for the surgeon
when the operation starts, an unexpected complexity.

"And if the operation is an emergency, then the difficulties are
far greater."

The pupils of Yerevan’s special school N9 staged a silent protest
action in front of the President’s residence on December 1. The
children were accompanied by their parents. The protesters refused
to utter a word. Chairman of the "Community and Right" NGO, Advocate
Samvel Lazarian presented their demands and concerns. "The government
doesn’t allow them to speak up!" he said.

Note that under a government decision the school of hard hearing
children must unite with special school N15 for deaf and mute. The
parents say their children have an ability to speak, they simply
have hearing problems and if the schools are united they will have
to use gestures.

"These children cannot get adapted to the new surrounding. They will
feel depressed in the new atmosphere," said Samvel Lazarian.

Over 117 children from different Armenian marzes, as well as from
Karabakh, Georgia, Javakhk and Russia, attend School N9.

The demonstrators were promised to be given a chance to pose their
demands during an upcoming meeting with the deputy Minister of
Education and Science.

"We hope that the problem will be addressed otherwise the parents
will go on a termless strike," ensured Samvel Lazarian.

European Union – Azerbaijan’s Last Hope?

EUROPEAN UNION – AZERBAIJAN’S LAST HOPE?

PanARMENIAN.Net
27.11.2008 18:02 GMT+04:00

/PanARMENIAN.Net/ The Azerbaijani public is disappointed in the United
States, Israel and in western countries in general, so burning of
U.S. or Israeli flag doesn’t make a stir, an Azeri expert said.

"There are three trends in the Azeri society: pro-western, pro-Russia
and Islamic. The most serious factor is Islam, which appears as a
force capable to regain Karabakh. Islamists say that the world powers
will pump out all Azeri oil and then leave Azerbaijan to the mercy of
fate," Arif Yunusov, head of department of conflictology and migration
at the institute of peace and democracy, said during ‘European Union
and South Caucasus – A Security Partnership?’ international conference
in Yerevan today.

"There is also a sense of defenselessness before Russia. So, European
Union is Azerbaijan’s last hope and it can help resolution of the
Nagorno Karabakh conflict," Yunusov said.

ARMENIA SUPPORTS IRAN’S CIVILIAN NUCLEAR PROGRAM,- IRANIAN ISNA REPORTS

Noyan Tapan

Nov 25, 2008

YEREVAN, NOVEMBER 25, NOYAN TAPAN. The Secretary of Armenian National
Security Council Arthur Baghdasaryan said here on Sunday that Armenia
supports Iran’s "civilian nuclear program," Iranian Students news
agency (ISNA) reported. Baghdasaryan made his remarks during his
visit with his Iranian counterpart Saeed Jalili.

The United States, Israel and their Western allies accuse Iran of
secretly developing nuclear weapons, but Iran insists that its nuclear
program is only for generating electricity.

He urged the expansion of ties between Iran and Armenia on a multiple
grounds of "economic, diplomatic, defense and security relations."

Meanwhile, Baghdasaryan also called on Iran to actively cooperate in
Caucasus security, the report said.

Caucasus suffers the two unsettled conflicts, one is between Armenia
and Azerbaijan, the other is Russia and Georgia. Iran has made several
attempts to have a role in the region’s hot issue.

"Iran’s basic policy seeks practical support for promoting regional
peace and preventing crisis," Jalili said, adding that "cross-regional
forces’ interference only creates crisis and regional issues need to
be settled only by regional countries," according to ISNA.

IMF SEES NO NEED OF ADDITIONAL SUPPORT TO ARMENIA FOR PREVENTING WORLD FINANCIAL CRISIS EFFECTS

ARKA
Nov 24, 2008

YEREVAN, November 24. /ARKA/. The International Monetary Fund
(IMF) sees no need to provide additional assistance to Armenia for
preventing the effects of the world financial crisis in the country,
IMF Deputy Managing Director Murilo Portugal told a press conference
Saturday. Yet, the IMF is ready to provide financial assistance to
the country when required, he said.

Portugal reminded that on November 17 the IMF Executive Board approved
extending SDR 9.2 mln or about US$13.6 mln to Armenia over the next
three years under Poverty Reduction and Growth Facility (PRGF) program.

"The main purpose of the program is to signal the IMF’s continued
support for Armenia’s macroeconomic policies and structural reform
agenda in the country’s transition to emerging market economy status,
thus facilitating Armenia’s integration into the world economy,"
Portugal said. Allocation of a smaller amount means that Armenia does
not need large funds anymore, he added.

Yet, Portugal pointed to certain vulnerable aspects of the Armenian
economy and the country’s major dependence on direct foreign
investments and private transfers from abroad.

As money inflow is coming mainly from Russia, further increase in
transfers is closely tied with economic development of the Russian
Federation, Portugal said adding that 70% of the direct investments=2
0and 75% of private transfers were from Russia in the first half of
the year.

According to him, possible negative processes in the Russian economy
will have negative impact on Armenia, but no downward tendency is
recorded in transfers today. Transfers are received also from other
countries, but problems may still occur, he said.

According to Armenia’s National Statistical Service, individuals
received a total of $1,205mln non-commercial funds through the
banking system of Armenia in January-September – an increase of 33.8%
or $304.2mln as compared with the level recorded in the same period
of last year.

Dickranian School Gets New Basketball Court

Los Angeles: Since Mr. Rudolph Rameh’s assignment as the new Athletic
Director of the TCA Arshag Dickranian School, the adage ‘Healthy Mind
in a Healthy Body’ has taken a new stance. Last year, Mr. Rameh
wasted no time to upgrade the physical education facilities of the
school to enhance the athletic program, at the same time injecting new
vigor among the students by establishing multilevel sports teams and
organizing intramural games.

The most recent of such improvements occurred when a section of the
school playground was resurfaced and painted to be transformed into a
full size basketball court, allowing the school to host visiting teams
and organize interscholastic competitions.

The ribbon cutting ceremony of the court took place on Friday,
November 14 at 4:00 P.M., attended by Mr. George K. Mandossian,
Chairman of the school Board of Trustees, members of the Athletic
Committee, and an enthusiastic crowd comprised of parents and
students. The ceremony was followed by the Dickranian Lions
challenging the visiting Pacifica Christian High School team into a
friendly game, which ended with the home team winning.

The event ended with a reception at the Walter and Laurel Karabian
Hall, where team members, coaches, and game watchers enjoyed light
snacks and soft drinks.

The TCA Arshag Dickranian Armenian School is a federally tax exempt,
Pre-K to 12th grade private educational institution serving the
Armenian community of Greater Los Angeles since 1981.
