The earliest documented form of nasal reconstruction dates to around 600 BC in India, where the physician Sushruta described rebuilding a severed nose using a cheek flap. It is considered to be one of the oldest forms of plastic surgery. Historical accounts describe nose amputation as a form of public punishment and humiliation, associating the nose with honor, reputation and social standing.
Cosmetic rhinoplasty, however, is much more recent. The first widely recognized description of a purely cosmetic rhinoplasty dates to 1887, when American surgeon John Orlando Roe described an endonasal procedure to correct what he called a “pug nose.”
Today, rhinoplasty is one of the world’s most popular cosmetic procedures. More than 1 million rhinoplasties were performed worldwide in 2024, according to the International Society of Aesthetic Plastic Surgery (ISAPS). About 60% were performed on people aged 18 to 34, and nearly three-quarters of patients were women. At the same time, 45,512 rhinoplasties were reported among patients aged 17 or younger, making it the most common cosmetic surgical procedure in that age group. The global rhinoplasty market was valued at more than $5 billion in 2025 and is projected to exceed $9 billion by 2033, according to market research estimates.
And then there is Armenia, where the nose is not just a nose. It can be a marker of identity, a family resemblance, and also, something people want to change. The traditional “Armenian nose” is typically characterized by its large size, prominent hump, and drooping tip. It is an emblematic feature of many nations in the region. While it could be a symbol of national pride, for some Armenians, especially young women, it has also become a source of insecurity.
This wasn’t always the case. According to plastic surgeon Karen Danielyan, the 1988 Spitak earthquake helped accelerate the development of reconstructive plastic surgery in Armenia, and surgeons later applied that expertise to cosmetic procedures and that is how the field took off.
Armenians are often made to feel ashamed of their facial features, even by those who share their ancestry. In 2002, Danielyan began holding an annual “Most Armenian Nose” competition, offering the winner a free nose job. The contest was a publicity stunt meant to bring new customers into his clinic and it worked.
It has also had a lasting negative effect on the way Armenians, especially women, view themselves. A feature that could be seen as distinctly Armenian, and maybe even a source of pride, was turned into something that needed to be fixed, not celebrated.
Many Armenian women use structural issues with their noses as a starting point for cosmetic surgery. Chronic nasal obstruction and a deviated septum can cause health issues such as sleep apnea, chronic fatigue, and brain fog which can be addressed without a cosmetic procedure. Today, many rhinoplasties performed in Armenia are septorhinoplasties, which fix both the internal breathing problems and the external shape of the nose.
Emma, 22, is preparing to undergo a functional rhinoplasty in Armenia. Her mother, who had the same procedure a few years ago, encouraged her to fix the shape of her nose as well, since she is “going under the knife anyway.” Even though Emma was bullied for her hooked nose as a child, she doesn’t believe there is anything to fix.
“As you grow older, you understand that whatever nose you have suits your face. I like mine,” she says. “When I was in middle school, I thought having a hooked nose was not pretty. I looked at people with straight noses and thought I wanted to get mine done like that as well. But now, as everyone is getting a nose job, I like mine even more and I don’t want to change it, because everyone has the same face, especially with the latest beauty standards.”
Large, hooked noses are rarely, if ever, celebrated before they are modified. Many young people, mostly women, are mocked by their peers for having a larger nose; but once they have surgery, they are told they looked “better before”. As a result, young women and girls undergo face-altering, essentially irreversible surgery because of outside influence, and many end up regretting it later in life. It is a double bind: people are pressured to erase their unique characteristics, yet they are not rewarded for it unless the result looks seamless.
A key factor in the rise of rhinoplasties is the growing influence of social media. Platforms like TikTok and Instagram are focused on appearance and often reward Eurocentric beauty standards, making it easy to compare yourself with influencers and celebrities. Their audiences become more conscious of their own facial features and aspire to unrealistic beauty standards. Influencers and celebrities have also grown more open about their plastic surgeries, sending the message that if they can have these procedures, so can everyone else.
The “Barbie nose job” is a cosmetic rhinoplasty that gives the nose an unnaturally narrow bridge, an elevated tip, and small nostril openings, achieving doll-like proportions. Social media influencers have widely popularized it in recent years. It prioritizes “perfection” over natural beauty and does not aim to go unnoticed. It also carries several risks: removing too much cartilage or bone can lead to breathing difficulties or even nasal collapse. Revision rhinoplasties are often needed to maintain the desired appearance, and they may require rib cartilage grafts.
Growing accessibility and transparency around rhinoplasty have made those seeking a Barbie nose job more aware of the risks of “going too far.” Increasingly, the emphasis is on results that look subtle, shifting the focus from unnatural, unachievable, ethereal beauty to an effortless and natural look. As a result, subtler, more “natural” procedures are now popular, and more patients are trading the Barbie nose job for a so-called “ethnic rhinoplasty.”
An ethnic rhinoplasty is designed to address structural and aesthetic concerns while keeping the nose in line with the patient’s heritage. Surgeons who perform it often say their goal is facial harmony: avoiding an over-operated look and preserving something of the patient’s ancestry. But there is a contradiction here. Ethnic rhinoplasty can make cosmetic surgery feel like a way of respecting one’s background while still changing the features that are most closely associated with it. It raises an uncomfortable question: if we have been taught to see certain ethnic features as flaws, does making them more acceptable through surgery really challenge those beauty standards—or simply find a more subtle way to conform to them?
Too often, Armenians choose to alter their faces rather than confront the prejudices that make them want to. It is easier to conform to Eurocentric norms and feel accepted than to find beauty in variety. Plastic surgery is a Band-Aid on the bullet wound that is self-hatred. Broadening our aesthetic appreciation, and coming to terms with the features that make us uniquely Armenian will solve more problems than rhinoplasty ever could.
